Preparing for New-Hire Orientation Day

To prepare in advance for filling out orientation-day forms on the spot, highlight and copy the list below right from your screen, then paste it into an email, word processor or text editor. You might lose a little bit of formatting, depending on the application to which you paste it. But it's easy to fix and will stay intact for the most part. It works well in Windows Notepad.

Do you prefer a nicer formatted version you may download in rich-text format (RTF), suitable for most word processors?

~ Insurance
Be prepared to provide the following for each of your beneficiaries and dependents.

* Beneficiaries
  - Name
  - Age
  - Address
  - Home and work phone numbers
  - Social security number
  - Percent of distribution
* Dependents
  - Name
  - Age
  - Address
  - Home and work phone numbers
  - Social security number

~ Federal W-4 and State Equivalent
You must fill out these forms before you can collect your paychecks, so your employer knows how many allowances (exemptions) to consider when withholding your Federal and state income taxes. Typically, the forms come with instructions.

* The IRS site (http://www.irs.ustreas.gov/prod/tax_edu/teletax/tc753.html) has more information for calculating your Federal allowances in advance. (Click on Publication 919.) To read and save the W-4 instructions and worksheet while online, first download the free Acrobat Reader (http://www.adobe.com/support/downloads/main.html) from Adobe.
* To calculate your state allowances in advance, get the form and worksheet from your state tax agency.
  - To link to your state tax agency online, see Tax Forms and Information (http://www.westga.edu/~library/depts/govdoc/tax.shtml) from the State University of West Georgia.
  - Alternately, try TaxTime (http://www.savewealth.com/taxforms/state/) at SaveWealth.com.
* The form your employer gives you might differ from the Federal and state forms, but the end results are the same. For example, your employer's form may include space for both Federal and state allowances.
* Tip: Your paycheck stub from your last employer typically includes the number of allowances you previously claimed.

~ New Employee Information
You might have to provide the following, even if you already did so when filling out your job application (http://jobsearchtech.about.com/library/weekly/aa041000.htm).

* Relevant paperwork if you have a work visa or weren't a citizen at birth
* One or two forms of identification even if you were a US citizen at birth, such as a
  - Driver's license with photo or other photo ID issued by a Federal, state or local government office
  - Social security card
  - Birth certificate
  - Passport
* Dates of US military service (for veteran equal opportunity programs)
* Full contact information of someone your employer can reach in case of emergency, including work and home phone numbers

~ Request for Automatic Deposit
You fill out this form if you want your employer to automatically deposit your pay in one or more of your bank accounts. Be prepared to provide the following for each account.

* Your account number
* Financial institution's
  - Name
  - Address
  - Phone number
  - Transmit routing number. Typically, it's the first nine digits in the string of numbers and odd-looking characters printed near the bottom of your personal checks. (The string includes the check number and your checking account number, and the nine remaining digits are the bank's transmit routing number.) But rather than guess, it's wise to verify the transmit routing number with your bank so your pay doesn't end up in the bit bucket.
* For automatic deposits to a checking account, bring a blank, voided check.

~ Prior Service
You fill out this form if you have worked for the company or its subsidiaries in the recent past, so you can "bridge" vacation and other benefits if you're entitled. Be prepared to provide information such as

* The name of the company or subsidiary
* Department or division
* Dates of employment
* The following at the time you left your job
  - Job title
  - Wage or salary
  - Work address
