Preparing for New-Hire Orientation DaySample Form Details
So that you may prepare, below is a list of the types of forms US employers typically require you to fill out on your first day. There are more forms that you'll probably have to complete, such as for retirement accounts and stock options, that are not included in the list. That's because your new employer will likely give you 30 days or so to fill out these additional forms. The same goes for insurance forms, because you'll probably be covered under default policies until you fill them out. But just in case your employer doesn't give you extra time for insurance forms, common "gotchas" for beneficiaries and dependents are included. Naturally, forms will vary from company to company. But if you prepare in the same way as you would for filling out a job application, plus prepare to provide the information in the list below, you'll be ready to go for most situations. The list excludes the ordinary for which you don't need to prepare, such as your contact information. Insurance Be prepared to provide the following for each of your beneficiaries and dependents.
Federal W-4 and State Equivalent You must fill out these forms before you can collect your paychecks, so your employer knows how many allowances (exemptions) to consider when withholding your Federal and state income taxes. Typically, the forms come with instructions.
