New Employee Information

You might have to provide the following, even if you already did so when filling out your job application.

  • Relevant paperwork if you have a work visa or weren't a citizen at birth
  • One or two forms of identification even if you were a US citizen at birth, such as a
    • Driver's license with photo or other photo ID issued by a Federal, state or local government office
    • Social security card
    • Birth certificate
    • Passport
  • Social security number
  • Dates of US military service (for veteran equal opportunity programs)
  • Full contact information of someone your employer can reach in case of emergency, including work and home phone numbers

Request for Automatic Deposit

You fill out this form if you want your employer to automatically deposit your pay in one or more of your bank accounts. Be prepared to provide the following for each account.

  • Your account number
  • Financial institution's
    • Name
    • Address
    • Phone number
    • Transmit routing number. Typically, it's the first nine digits in the string of numbers and odd-looking characters printed near the bottom of your personal checks. (The string includes the check number and your checking account number, and the nine remaining digits are the bank's transmit routing number.) But rather than guess, it's wise to verify the transmit routing number with your bank so your pay doesn't end up in the bit bucket.
  • For automatic deposits to a checking account, bring a blank, voided check.

Prior Service

You fill out this form if you have worked for the company or its subsidiaries in the recent past, so you can "bridge" vacation and other benefits if you're entitled. Be prepared to provide information such as

  • The name of the company or subsidiary
  • Department or division
  • Dates of employment
  • The following at the time you left your job
    • Job title
    • Wage or salary
    • Work address
